Irene is seen having sex with a guy at a motel room. She is lying on her back with her legs straight up in the air, then rolling over to ride him as they continue to have loud sex. After they finished having sex, Irene seen kneeling on the bed while guy is getting dressed up. Then she stands up as she argues with him about filming there sex. The guy ignore her leaves the room. She grabs a gun and follow him. She is seen walking in front of the motel naked to her heels. She confronts the guy, point the gun at him. But she gets shocked when the guy get killed by a unknown man. She's runs back to the motel room and locks the door. Viewers get to see every inch of her naked body as she tries to find somewhere to hide. Although she hides underneath the bed, the killer finds her and kill her. The woman's breast, butt, and completely shaved vagina are shown frequently throughout this lengthy scene.
Later, her bare body is shown in an empty bathtub as her outer and inner labia are clearly seen.
Axel (and therefore moviegoers) views a sex tape (Irene's) that is crime evidence.
In other scenes we hear couples having boisterous sex off-screen.
Axel has an affair with one of Sarah's co-workers, getting her pregnant. (She's seen getting dressed after one of their trysts and there is a closeup shot of her crotch in very tight panties - viewers see her cameltoe in closeup as she turns towards the camera).

We see a hospital filled with dead very bloody bodies from a distance, some bodies are missing limbs while a couple are cut in half (from a distance) we see the body of a woman whose chest is open (graphic and bloody) we later see her heart on a table. The walls are covered with blood.
A villain hits a young man in the back of the head with a pick-axe; his eye pops out and stays on the pointed side of the axe.
A woman is hit with a shovel in the mouth, the shovel is later pushed cutting her head from the jaw-up (brief but very graphic/bloody)
Several people are hit with an axe in several scenes, sometimes it's off-screen but most of the time it's graphically on the screen with lots of blood and gore usually resulting in mutilation.
We see a very bloody/gory dead woman's body, her whole body is bloodied up. Her chest is opened up and her legs are spread apart and her own heart is lying between her legs,words are written in blood on the wall above her head.
-A man's head is impaled with a pick axe. Blood runs all over his face, the axe is later pulled, blood splatters all over the place as his body drops to the ground.
A small woman is hit with a pick axe, the villain thrusts it up through her head from her jaw upwards and then lifts her up into the lights and electrocutes her.
The villain takes many swings at a woman with the pick axe through a hole in the spring. She misses them all. He then takes the axe in her stomach and goes upward. Off-screen but blood splatter on the wall.
Many human hearts are seen throughout the movie, sometimes we see several bodies cut open as well.
A man is pushed towards an axe setting on the floor, the axe impales the man in the eye, and lots of thick blood squirt on the floor.
A villan with a pick axe stabs an old man under his chin, it comes out of his mouth. The villain then pulls the axe out and it rips the man's chin/mouth off, it flies towards the screen in 3D and blood is splattered.
We see a dead burnt body of a woman, her skin is melted all over her face (brief but very disturbing)
A car crash scene results in two people being mildly injured with one having some blood over face.
Several very graphic and bloody stabbings with a pick-axe to the torso, limbs and head in several scenes. sometimes people fight with fists/kicks before they go down.
