People are killed in explosions, but they are not very graphic.
Calamity Jane impales a man with the horns of her "bull" car, this scene is gory but it is 1 second.
Cars ram each other throughout the movie, in an attempt to kill one another, this scene is not gory.
A man gets impaled in the groin by the front dagger of Machine Gun Joe Viterbo, this scene is gory.

People are hit by cars throughout the movie. Sometimes it isn't very graphic, but often other times it's highly gory, showing bloody impalings, splattered bodies and crushed heads.
At "Mercy Hospital", a group of old people are on road on their beds or wheelchairs, probably left on the road to get rammed. However, the driver who is Frankenstein who have a heart for the elderly, runs over all the female nurses on the sidewalk. Offscreen, but we are able to see the nurses fly around and scream while a doctor and two nurses on the second floor glare the scene.

Despite the film's crude and highly gruesome nature, much of it is actually rather lighthearted. Undoubtedly, some will still find the race scenes a bit intense, as well as the fact that old people and children are considered targets.
